EGCSD is recipient of Model Transition Project Grant Award

The EGCSD Special Education department recently wrote a proposal for a Model Transition project and received a grant award from the NYS State Education Department totaling $172,040 over the next 3.5 years.

The project will improve transition planning and services to special education students at Columbia High School that will assist students as they move from high school into post-secondary education or employment. 

Sandy Sweet-Perpetua from Northeast Career Planning has been selected as our Transition Coordinator and will be working at Columbia High School Monday-Thursday.  She will be assisting our juniors and seniors for preparation for life beyond high school including:

  • Helping students and their families with the VESID application process
  • Recommending day and residential habilitation programs
  • Connecting students to job coaching services
  • Linking students entering college to the disability centers on campus
  • Creating a Transition Resource Guide for parents, students and staff
  • Provide training on transition related topics to parents and staff
We will also be implementing a driver education program for students with disabilities who may have difficulty accessing our general drivers education program in its typical format.
